  • Catholic Charities of Buffalo A comprehensive human service provider serving all eight counties of WNY, working to protect, strengthen and empower more than 130,000 people each year. Headway of WNY, Inc. A support and advocacy agency for persons with brain injuries and other disabilities. Jewish Family Services Offers a broad range of complementary, integrated services delivered by highly qualified professionals in psychiatry, psychology, nursing, social work, and rehabilitation counseling for people of all ages including seniors.

  • Meals on Wheels of America Provides community-based Senior Nutrition Programs throughout the U.S. There are a number of Meals on Wheels organizations in Western New York that can be found here.
  • People, Inc. Offers a variety of services including residential, employment, community outreach, health care, and recreation programs for seniors, families, and people with disabilities to enable them to live more healthy, independent, and productive lives.
  • Catholic Health/Living Independently for Seniors (LIFE) A program of all-inclusive care for the elderly, (PACE program) to help them remain in their home but qualify for nursing home care. Provides assistance to help seniors maintain their health, complete day-to-day activities, and remain safe for as long as possible.
  • Weinberg Campus/Total Aging In Place Program A managed long-term care health plan created as a way to provide flexibility to meet the health care needs of seniors and enable them to continue to living safely in the community. Programs include a social day program, adult day health program, care management, medical transportation, and home care services.
  • Hospice Buffalo Provides medical care, emotional and spiritual support to people and their families facing life-limiting illness in homes, hospitals, adult facilities, nursing homes, group homes, and hospice residences.

Geriatric Care Managers
  • Geriatric Care Managers of WNY Provides a comprehensive assessment of seniors including the person, the place, health status, documents, and the finances, and assist in locating the most appropriate rehabilitation setting, Assisted Living, Skilled Nursing facility, or home.

Adult Day Care - Social
  • Lord of Life Adult Care Center A social day program for adults, providing activities, nursing, nutrition, transportation, personal care and caregiver services.
  • Amherst Adult Day Care Provides adult day care services including organized daily activities in a community-based setting, transportation, meals, and professional supervision.
  • DeGraff Adult Day Care Provides adult daycare to meet the needs of the chronically ill and impaired adult (ages 18 and above) who live in the community and includes socialization, assistance with personal care, and a wide variety of activities.
  • Aurora Adult Day Services Provides adult daycare for functionally impaired adults who are unable to be left alone and to offer respite to their caregivers.

  • Catholic Health System Provides medical adult daycare offering a place where someone requiring supervision can spend the day and have the opportunity to socialize and receive medical care.
  • Schofield Adult Day Health Care Provides physical, occupational, and speech therapies, nursing care, personal care, case management, and other services in a supportive group environment.
Home Health Care - Non-Medical/Companion Care
  • Home Instead Senior Care A network of locally owned franchises offering in-home care to help families keep aging loved ones in their homes as they grow older.
  • Touching Hearts at Home Helps seniors, people with disabilities, and those who are battling illness to preserve their quality of life and remain independent in their own homes by providing non-medical companionship, transportation, and homemaking services.
  • Sitters on Standby Provides in-home companionship and services to meet the needs of seniors including, but not limited to, light housekeeping, shopping, errands, and medical reminders.

Skilled Nursing

A skilled nursing facility is a type of nursing home recognized by the Medicare and Medicaid systems as meeting long-term health care needs for individuals who have the potential to function independently after a limited period of care. A multidisciplinary team guides health care and rehabilitative services, including skilled nursing care.

Licensed Continuing Care Retirement Community
New York State approved Continuing Care Retirement Communities (CCRC) are housing and care complexes restricted to seniors. They include independent living units (apartments and/or cottages), social activities, meals, supportive assistance, and personal care all on one campus. Nursing Home care is also included and may be provided on or off-campus. Residents’ housing and health care are covered under a life-care contract or under a long-term housing and health care contract. The Community’s health-related and nursing care are licensed and regulated by the New York State Health Department, and the Community’s financial status and pricing structure are overseen by the New York State Insurance Department.
